Request for Expert Witness Funding Pursuant to La. R.S. 15:142(f)
The Private Counsel Affidavit Request Form must be completed in full and submitted to the Office of the State Public Defender (OSPD) before any action or review can take place. Submit via email at info@statepublicdefender.la.gov address to State Public Defender Rémy Starns and Deputy State Public Defender Michael Mitchell.
